How to File a Claim

Claims and customer service are administered by our third-party administrator, ACI – Administrative Concepts, Inc.

To submit a claim, employees may contact ACI’s customer service line at 888-346-9905.

When filing a claim, please include:

  • An itemized bill for the services received
  • The primary Explanation of Benefits (EOB)
